Noteworthy successes and key engagements:

  • Launched the Theon® platform as a service (PaaS), integrated analytics and population health platform—designed to drive financial, quality, and health outcomes
  • Partnered with a global analytics leader to power our leading integrated analytics and population health platform
  • Achieved full, three-year National Committee on Quality Assurance (NCQA) Case Management Accreditation and Patient and Practitioner Oriented Disease Management Accreditation for the asthma, coronary artery disease, heart failure, chronic obstructive pulmonary disease and diabetes programs. As of August 16, 2016, only 32 organizations are accredited for standalone complex case management and 18 have achieved this distinction for standalone disease management services.
  • Initiated regional accountable care arrangements to 30+ physician practices and increased patient engagement rates from industry standard of 20% to 75%
  • Partnered with a large health insurer in Pennsylvania to support five accountable care arrangements through our Theon PaaS
  • Formed a strategic partnership with a global healthcare products company to target chronically ill patients for outreach through Geneia’s @Home remote patient monitoring program
  • Structured an educational program between The Geneia Institute and a well-known College of Nursing located in Pennsylvania to educate healthcare professionals on the challenges of value-based care delivery
  • Commenced a partnership between The Geneia Institute and Massachusetts Hospital Association (MHA) to provide skills-based training to its member base of 79 hospitals and health systems
  • Engaged with a $50+ billion global security and information technology company on a feasibility study to revolutionize the treatment of high cost disease states (see Data Analytics – Identify Illness Before Your Body Does)
  • Directed a 10-year initiative with an academic healthcare institution to drive health care value, wellness and engagement
  • Created and administered statewide medical home and accountable care organization initiatives for more than 100,000 patients
  • Implemented a private exchange software platform and product options concurrent with state and federal exchange activities
  • Conducted advanced training and consulting to a large health insurer in California serving 3.3 million members, throughout their transition to a new claims processing system
  • Engaged with a large health insurer in Iowa serving over 2 million members, to lead their transition to a new claims processing system
  • Contracted to provide care coordination to 750,000 members through Geneia’s clinical programs
