Jody’s health system used the insights in the Theon® analytics platform to: 

  • Reduce out-of-network referrals by five percent in the first year,
  • Save nearly $143,000 on emergency department costs, and
  • Lower hospital admissions with a remote patient monitoring program and other provider and patient outreach.

To help you know that you too can replicate her success, let me tell you about Jody and her health system. 

Jody works for the chief medical officer of a health system comprised of three hospitals, 25 primary care practices and nearly 40 specialty practices of varying sizes. Jody’s health system participates in two accountable care organizations (ACOs): one for its Medicare population and one for a large percentage of its commercial patients. They also have a number of value-based contracts with commercial payers.

The health system’s CMO charged Jody with creating initiatives to quickly recoup its investment in the Theon® analytics platform but, just as importantly, to achieve the goals in the system’s value-based contracts in the early years and position them to continue to succeed in subsequent years.

Like virtually every health system, Jody’s had low hanging fruit. She used the Theon® platform to identify hers – a severe leakage problem - and to design a solution: a comprehensive physician and patient education program, a new incentive for in-network referrals and the addition of in-network gastroenterologists.
