The award recognizes Salesforce partners who serve as a partner role model delivering healthcare applications integrated with Salesforce Health Cloud that improve the efficiency and effectiveness of the patient care cycle.

The Geneia - Salesforce Health Cloud solution combines the Theon® platform’s cost and quality analytic capabilities with Health Cloud’s workflow, member engagement and communication tools. Available on Health Cloud, the Theon® platform drives repeatable improvements in health, quality and financial outcomes.
The Theon® platform has helped customers:
- Close care gaps and increase Medicare star ratings
- Reduce acute inpatient hospital admissions by up to 7.2 percent
- Lower emergency department visits by more than 8 percent and readmissions by more than 14 percent, and
- Reduce claims costs by 2 percent
