Remote Patient Monitoring is a Wise Investment for Health Plans

Joined by executives from Medtronic, Ingerick will discuss the opportunity health plans have to improve the cost, quality and experience of chronically ill members by effectively integrating remote patient monitoring into their existing care management programs. The session, Outcomes of an Integrated Remote Monitoring Pilot, will take place on October 26 at 11:50 am.

Geneia recently completed a year-long remote patient monitoring study of heart failure patients. Health plan members in the study group were selected for the program using predictive analytics, and then given state-of-the-art technology and personalized clinical support. The heart failure patients in the remote monitoring pilot fared much better than the control group, experiencing:

  • A net 45 percent reduction in acute hospital admissions,
  • A stabilization of risk scores indicating a slowing of disease progression, and
  • A 37 percent increase in medication adherence.

In addition to significant outcomes improvement, the health plan yielded an annual savings of $8,375 per monitored patient, which, for a typical one-million-member plan could translate into about $1.1 million saved per year.
