Population Health Alliance: Using AI to Improve Population Health
Models created using artificial intelligence (AI) are increasingly being used by physicians, hospital and health plans to predict and prevent unnecessary exacerbations of chronic illness. In this webinar, Geneia chief analytics and technology officer Fred Rahmanian will discuss how these models are developed and review models such as one that predicts which type 2 diabetes patients will experience a diabetes-related complication in the next 12 months, another that predicts which heart failure patients will have complication(s) and a third that identifies patients at high risk for severe COVID-19 impacts.
Geneia’s vice president, consulting services and clinical innovations Shelley Riser will share best practices for putting analytic models into population health practice. She’ll also discuss how she and her team are using a hypertension model to create a more effective, care management program.

Salesforce Blue Success: The Payer Post-Pandemic Reality: Social Determinants, Id and Strat & Member Engagement
In the past two months, healthcare utilization has plummeted. Preventive care has declined precipitously as has emergency care for heart attacks and strokes.
As but one of many examples, a Commonwealth Fund study found “the number of visits to ambulatory practices declined nearly 60 percent in mid-March and has remained low through mid-April.” Contemporaneously, 61 percent of patients report they have been adversely impacted by social determinants of health (SDoH) during the pandemic, a number most experts expect to rise. At the same time, early research has suggested that people with comorbidities like diabetes, hypertension and heart disease are at higher risk for COVID-19 complications and hospitalizations.
These realities put a premium on payers using identification and stratification (id and strat) that integrates social determinants and analytics revealing the members most likely to experience severe COVID-19 impacts. In this webinar, Geneia president and CEO Heather Lavoie and Geneia analytics and technology officer Fred Rahmanian will demonstrate how payers can use enhanced id and strat and member engagement strategies, including the digital front door, to identify and outreach to members with the greatest risk and the most significant gaps in care. They also will discuss payer post-pandemic strategies to determine and engage members in the right dose of care in the most appropriate setting.
